Generally, to qualify for FSA services, most individuals need to complete the intake process and meet with a case manager. The purpose of this comprehensive case management approach is to identify other needs the client may have that FSA can assist with in order to help each individual reach their personal goal of self-sufficiency.
Qualifications
- Background Requirements:
- Families with children 18 and under
- Elderly (55+)
- Disabled (Wrap Around Services)
- Must meet low-income guidelines. (For Guidelines - Click Here)
- Verification paperwork, including but not limited to:
- IDs for all household members
- For all children in the household, you will need to submit either birth certificates, social security cards, shot records, school records, or Medi-Cal cards in place of an ID.
- Proof of address, this can include any of the following:
- This can be electric, gas, or telephone bills, with receipts from the last 2 months in your name with your current address.
- If you are living with someone else, a letter clearly stating who you are, your address, and how much you pay in rent.
- If you are living in a motel, a receipt identifying you, the motel address, the amount you pay, and receipts for any payments.
- IDs for all household members
